We know your time is valuable! To make sure that we are providing you with the very best care, we ask that you fill out and submit your New Patient Paperwork (Patient Questionnaire & Patient Medical History Forms) at least 48 hours prior to your first appointment with us. If you are scheduled for a surgical consultation, we recommend that you bring someone to your appointment that can assist you in making important medical decisions. This person should also be able to drive in case you do not feel comfortable driving when your eyes are dilated. It is also helpful to have your calendar available so that we may schedule your surgery at a time that is convenient for you. If you are unable to keep your appointment, please call our office to cancel so that the time may be given to another patient. Cancellations must be made within 24 hours of your scheduled appointment.

When you arrive at our office for your appointment- simply walk up to our kiosks located to the right of the front desk (one of our staff members will be there to guide you every step of the way).
Please have your driver’s license and insurance card ready when signing in at the kiosk screen.
The kiosk will go through a series of seven questions and ask you to scan your driver’s license and insurance card. Upon completing this simple check in process, the kiosk will tell you exactly what your current deductible is, as well as any co-payment that is due.

While the length of patient visits vary, the initial visit is usually quite extensive — so please be prepared to spend at least two hours with us. While you wait, consider visiting our Patient Education Area, enjoy complimentary cookies, water, coffee and take advantage of our free Wi-Fi access. Map & Directions

You will receive a call with your exact arrival time the day before your scheduled surgery. If you have scheduled complimentary transportation service, you will receive a separate call letting you know what time transportation will arrive at your home.

The center holds the distinguished certification of the Accreditation Association for Ambulatory Health Care (AAAHC), indicating that quality care and personalized service are continually delivered. To qualify for this premier accreditation, rigid standards must be met in patient safety, emergency equipment, anesthesia, instrumentation, policies, procedures and quality control.
