Your Office Visit

Thank you for choosing Collins Vision for your eye care needs. To help you feel prepared, we’ve listed what you need to do before your appointment, what to bring with you, what to expect during your visit, and how to locate our offices. We ask that you read through this page in its entirety before arriving at your appointment.

New Patient Paperwork

We know your time is valuable! To make sure that we are providing you with the very best care, we ask that you fill out and submit your New Patient Paperwork at least 48 hours prior to your first appointment with us. A Checklist For Your Visit

  • Your insurance and/or Medicare cards
  • Current photo ID
  • Completed New Patient Paperwork (if you are unable to submit by using the Online Patient Registration)
  • Name and telephone number of your pharmacy
  • Name, address, and telephone number of the doctor who referred you to our office, if applicable
  • Prescription eyeglasses (even if they no longer correct your vision)
  • List of current medications
  • Your personal calendar (if you are scheduled for a surgical consultation)
  • Sunglasses (in case your eyes are dilated)

If you are scheduled for a surgical consultation, we recommend that you bring someone to your appointment that can assist you in making important medical decisions. This person should also be able to drive in case you do not feel comfortable driving when your eyes are dilated. It is also helpful to have your calendar available so that we may schedule your surgery at a time that is convenient for you. If you are unable to keep your appointment, please call our office to cancel so that the time may be given to another patient. Cancellations must be made within 24 hours of your scheduled appointment.


Your Check-in Process

Our kiosk allows us to streamline the often-cumbersome process of paperwork and lets us focus on you, our patient.

Our new kiosk will help:

  • Provide a better patient experience
  • Increase the time interaction we have with you
  • Reduce paperwork for everyone – patient and staff
  • Shorten your waiting room time
  • Provide in real time information regarding your co-pays, deductibles, and other insurance information

When you arrive at our office for your appointment- simply walk up to our kiosks located to the right of the front desk (one of our staff members will be there to guide you every step of the way).

Please have your driver’s license and insurance card ready when signing in at the kiosk screen.

The kiosk will go through a series of seven questions and ask you to scan your driver’s license and insurance card. Upon completing this simple check in process, the kiosk will tell you exactly what your current deductible is, as well as any co-payment that is due.

Length of Your Visit

While the length of patient visits vary, the initial visit is usually quite extensive — so please be prepared to spend at least two hours with us. While you wait, consider visiting our Patient Education Area, enjoy complimentary cookies, water, coffee and take advantage of our free Wi-Fi access.  Map & Directions

Your Upcoming Surgery

The following information will assist you before and after your surgery at St. Marks Surgical Center. Should you have questions specifically regarding your medications, please call us at 239.936.4706.

Surgery Arrival Times

You will receive a call with your exact arrival time the day before your scheduled surgery. If you have scheduled complimentary transportation service, you will receive a separate call letting you know what time transportation will arrive at your home.

Please note that it is customary to pay for services rendered at the time of your visit including any co-payments, co-insurance and/or deductibles unless other arrangements have been made prior to your scheduled appointment.

Medical Insurance

We accept most major insurances and as a courtesy to our patients, we file claims to primary and secondary insurances. Please check with your insurance company to verify that we are a participating provider and no prior authorization or referral is needed.

Vision Plans

We accept VSP and most EyeMed plans, which offer affordable eye exams and discounts on glasses and contact lenses. Please check with your provider to verify coverage.

Interest-Free Financing

At Collins Vision, we believe financial considerations should not hinder your desire to obtain your best visual outcomes. If you are considering Laser Vision Correction Surgery or Cataract Surgery with Premium Lifestyle lenses, we offer interest-free financing through both Alphaeon and Care Credit.
