PREPARING FOR YOUR APPOINTMENT

Each of our appointment types is unique, so we’ve laid out details below to help you prepare specifically for your upcoming visit. How to Prepare

  • Please plan to be in our office for about an hour.
  • Please be sure your patient portal is filled out completely or update any information that might have changed.
  • Please discontinue soft contacts for one week before your appointment. Hard and gas-permeable lenses should be discontinued for two weeks. If for some reason this isn’t possible, please let us know!
  • Please plan to arrive 15 minutes prior to the appointment time.


What to Bring

  • Your Cataract Surgery Folder
  • Your insurance and/or Medicare cards
  • Current photo ID
  • Prescription eyeglasses (even if they no longer correct your vision)
  • A list of all current medications
  • A list of any medical specialists that you see (ie. cardiologist, etc)


What to Expect

This is one of our favorite visits with you because there’s a ton of great information to share! During this appointment, we will:

  • Take a few important diagnostic measurements
  • Review our comprehensive list of frequently asked questions – this is the time to take notes and ask questions of your own, too!
  • Discuss the Lifestyle Questionnaire you filled out prior to your appointment
  • Review your medical history, medications and an anesthesia form to determine if any clearances for surgery may be necessary
  • Introduce you to our state-of-the-art vision simulator
  • And finally, introduce you to the Collins Vision Cataract Surgery Paths

How to Prepare

  • Please watch the video from Dr. Collins below in its entirety.
  • Please plan to be in our office for about two hours.
  • We offer complimentary transportation to and from St. Mark’s for your surgery day if needed. Please be thinking about whether or not you will need to use this service.
  • Please be thinking about whether or not you’d like to be dependent on glasses after surgery.

  • Please look at your calendar and have some surgery dates in mind.
  • Please have a good understanding of your insurance benefits and how much they will cover.

What to Expect

Now that you’ve had all of your testing and learned the basics, it’s time to meet with your surgeon and your counselor. At this appointment, you will:

  • Visit with a technician who will dilate your eyes and perform a basic eye exam
  • Learn your surgeon’s recommendation for your surgical plan
  • Make a final decision about which path you’d like to choose
  • Review your surgical benefits with your counselor
  • Discuss financing options with your counselor
  • Schedule your History and Physical (H&P), surgery and post-operative appointments
  • Arrange your complimentary surgery-day transportation if needed.

How to Prepare

  • Please plan to be in our office for about an hour.
  • Please plan to arrive 15 minutes prior to the appointment time.
  • Please be sure your electronic consent forms have been signed and submitted.
  • Please be sure all of your questions have been answered and if not, you’ve written these down to discuss with the nurse practitioner.
  • If you’ve had any changes to your medications, please be sure to have the names and dosages handy or update them in your patient portal.
  • Please have a payment plan in order as all surgery fees are due at this appointment.


What to Bring

  • Notes regarding any questions you have
  • Notes regarding any changes to your medications
  • Credit card or checkbook to pay for your surgery fees


What to Expect

This is your final step before cataract surgery! At this appointment, you will meet with our nurse practitioner to:

  • Review your medical history to confirm all information is correct and that you’re set for surgery
  • Review your pre and post-operative instructions
  • Review the details of surgery day
  • Collect all payments for surgery

What to Expect

The YAG consultation is to determine the medical necessity of performing a YAG Laser Capsulotomy after cataract surgery. At this appointment, you will:

  • Have an exam to determine whether a YAG is necessary
  • Meet with your cataract surgeon to confirm your diagnosis and discuss the next steps
  • Meet with a counselor to schedule your procedure date

How to Prepare

  • Please plan to arrive 15 minutes prior to the appointment time.
  • Please be sure your patient portal is filled out in its entirety or update anything that has changed.
  • Check the Forms section in your patient portal; you should see a Lifestyle Questionnaire; please be sure to fill that out!
  • Please continue any eye drops you are currently using.
  • Be thinking about what you want to achieve with your surgery and jot down any questions you have about the procedure.
  • Please note that you may or may not be dilated for this visit. If dilation is a concern for you, please let us know ahead of time by calling 239.936.4706.

  • Conduct a thorough exam of your eye health, vision and eyelid area
  • Discuss your concerns to determine what course of treatment is right for you
  • Have you meet with a counselor to discuss recommendations, pricing and insurance coverage. They’ll also schedule your procedure(s)!

What to Expect

The purpose of this appointment is to address any medical concerns related to your eye(s). You will undergo a series of diagnostic testing to determine if any conditions are present or have worsened. At the conclusion of an eye exam using your medical insurance, you will not be given a prescription for your glasses or contacts unless you’ve opted to have a refraction performed.

A good rule of thumb about refraction fees: If your vision has changed due to a known condition, there will not be a charge for your refraction. However, if you’d just like to have your glasses prescription updated during your eye exam, you may have to pay a refraction fee.

  • After seeing your doctor, plan to visit with Mark, our licensed optician, in our Optical Boutique for a customized glasses fitting and to explore the latest lenses and frames.

What to Expect

The purpose of this appointment is to check your vision to see if anything has changed since your last appointment. You will have a number of routine screening tests performed to ensure that all is well and will receive a new prescription for your glasses or contact lenses at the conclusion of your appointment. How to Prepare

  • Please discontinue use of contact lenses per the following guidelines –
    • Soft Daily & Extended Wear – one week prior to your exam
    • Toric Lenses – two weeks prior to your exam
    • RGP & Hard Contacts – three weeks prior to your exam
    • Note: Wondering why we have you remove your contacts? It helps us get the most accurate measurements for your customized LASIK or ASA procedure. If for some reason you’re unable to remove your contacts for the time periods outlined above, that’s okay! It just means that we might have to squeeze in another re-testing appointment after you’ve been able to remove your contacts.

  • Current photo ID
  • Calendar to schedule your surgery
  • Prescription eyeglasses (even if they no longer correct your vision)
  • Sunglasses to protect your eyes because you will be dilated during this appointment

What to Expect

Congratulations! You’re taking the first step toward a life without glasses or contacts. We’re so excited to help you explore your options and achieve your vision goals. During your comprehensive LVC consultation, you will:

  • Meet with a technician who will perform a comprehensive eye exam using an autorefractor, phoropter and Pentacam
  • Meet with your surgeon who will personally examine your eyes, review your test results, discuss your personal goals for your vision and then present his top recommendation
  • Meet with a counselor who will recap your visit and recommended treatment plan, help you understand your potential out-of-pocket costs and financing options, as well as help you with the scheduling process

ADDITIONAL INFORMATION

COVID-19 Safety Precautions:

Beginning in March 2022, we will no longer be requiring masks. If you choose not to wear a mask, please be mindful of those around you.

We will be fully equipped to quickly put on a mask at your request and can provide one for you if needed.

Cancellations:

If you are unable to keep your appointment, please call our office to cancel so that the time may be given to another patient. Cancellations must be made within 24 hours of your scheduled appointment.
